Department of Immunohematology

The department is included in the Complement of the laboratories of the IHBT. It continuously advocates an independent and impartial assessment of the professional competence of the Complement of Laboratories according to the ČSN EN ISO 15189 standard, the application of which is verified by the Czech Institute for Accreditation. The test methods within the scope of accreditation are defined by the Annex to the Certificate of Accreditation in the current version, which is available on the IHBT website.

The work of the department is divided into two parts:

  • routine part - immunohematology department with local scope
  • a specialized part with national and international scope - NRL for immunohematology

The Immunohematology Department performs basic immunohematological testing of blood donors of the local blood transfusion department.

Routine pre-transfusion testing is provided for patients of the clinical department of IHBT, possibly for patients of paediatric haematology at Motol Hospital.

In patients with antibodies, their specificity is determined, erythrocyte typing is performed, possibly platelet typing to ensure the optimal blood product for these sensitized individuals.

The chimerism of erythrocytes after bone marrow transplantation is monitored, especially with regard to the early detection of the change of the recipient's blood group to the donor's blood group - again to ensure the correct blood product for these patients.

The department also detects or specifies platelet and leukocyte antibodies in patients in whom the cause of low numbers of these elements is sought.

As part of TRALI prevention, HLA antibody screening is performed in blood donors of the TÚ IHBT. In addition, serological tests for the detection of HIT type II-associated antibodies (anti-heparin/PF4) are performed.

The NRL for immunohematology performs the most demanding immunohematological examinations when dealing with complex cases of antibodies and antigens sent to the laboratory from all over the Czech Republic and Slovakia and participates in the preparation and supervision of external quality control cycles.

The Immunohematology Department is involved in research projects (Research Plan of the IHBT). Its staff is also involved in undergraduate teaching in the field of immunohematology (within the IHBT) and postgraduate (ILF, NCONZO), and at the same time they improve their own qualifications by participating in training events.

The staff is bound by confidentiality according to the Health Services Act, 372/2011 Coll.

Confidence and high data security is one of the basic priorities within the activities of the Complement of Laboratories of the IHBT. The established procedures are carried out in accordance with the Personal Data Processing Act 110/2019 Coll. and Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC.
