Donor Hemapheresis
Department of Apheresis of the Institute of Hematology - donor hemapheresis
The Apheresis Department performs donor haemapheresis and prepares platelets from apheresis and allogeneic peripheral haematopoietic cell products for selected patients, as requested by the Clinical Department of the Institute of Haematology and other medical institutions.
Platelets from apheresis
- Deleukotized products in most cases in a replacement solution with a lower risk of post-transfusion reactions
- Our platelet donors have HLA traits tested, we can prepare platelets for a selected patient according to HLA traits (HLA selection), which is intended for patients refractory to platelet transfusions or patients with the presence of HLA antibodies. These special preparations must be ordered at least one day in advance, an "HLA selection" application form must be sent to our department, and a blood sample from the patient must be obtained for a compatibility test - the lymphocytotoxicity test (LCT test). In these special situations, HLA compatibility overrides AB0 compatibility and ABO incompatible platelets from apheresis can be given. In these cases we recommend administration under increased supervision or with premedication, or for more information contact here.
- Our donors are screened for anti-A and anti-B titers by blood type, which allows us to select donors with "low titer" anti-A, anti-B in the event of ABO incompatible administration in the event of HLA selection. This procedure further reduces the risk of possible post-transfusion reactions.
- A small group of our donors have been tested for HPA traits (Human Platelet Antigens) on platelets and we can select donors in special situations such as neonatal alloimmune thrombocytopenia (NAIT) to prepare platelets from apheresis for neonatal transfusion according to HPA traits (HPA selection). In most cases, these are urgent special situations and we recommend ordering these preparations by telephone and discussing the necessary procedure with the department staff
- for all platelet apheresis donors we perform laboratory check-ups (KO+diff., FW, biochemistry including ELFO, urine+sediment) once a year, ECGs are performed once every two years for donors under 40 years, once a year for donors over 40 years
- for donors who have been pregnant, we test for the presence of HLA antibodies before inclusion in the donor register for device donations
Peripheral blood stem cells (PBPC) allogeneic
- we cooperate with the Clinical Department of the IHBT and the Clinic of Paediatric Haematology and Oncology of the Motol University Hospital for PBPC donations of related donors and with the Czech Hematopoietic Cell Donor Registry (CSCR) in Prague for PBPC donations of unrelated donors, contact us here
- we assess the eligibility of donors before and on the day of donation, we perform the collection of peripheral blood stem cells (PBPC) and release the prepared PBPC product for further processing according to the legislation in force
- donors are monitored for a long time after donation in our department or in cooperation with general practitioners.
- Checks are carried out at regular intervals. The first time one week after donation, 6 and 12 months after donation and then once a year for 10 years after donation. Donors are invited by letter. A range of laboratory recruitment for doctors can be found here.
